A 61 year old male came to casuality with c/o abdominal pain on right side and constipation since a day
HOPI -patient was apparently alright 3 days ago when he developed pain in the right hypochondrium of the stomach which was sudden in onset and progressive in nature, which increased after eating oily food and decreased on taking medication.Associated with radiation of pain to the back.
The patient has no complaint of vomitings
History of constipation since a day. He is now passing flatus stools (white in colour)
Patient is a known alcoholic since 20 years with his last drink 3 days ago
PAST HISTORY :K/C/O hypertension since 9 years on medication of TAB. olmesar (olmesartan)
Not k/C/O DM, TB, asthma, CAD
No SOB
O/E - Conscious coherent and cooperative
Pallor :No
Cyanosis :No
VITALS :
Bp: 130/90 bpm
Respiration: 14min
SpO2:98%
GRBS: 138mg%
Drain : 400 my (Bilous)
P/A : Soft tenderness at drain site
Provisional diagnosis:
On USG it showed obstructive jaundice secondary to Acute calculous cholecystitis
Treatment:
INJ. TAXIM IV/BD
INJ. METROGYL 500mg/IV/TID
INJ. PAN40 mg/IV/BD
INJ. ZOFER 4gm /IV /OD
INJ. Tramadol 1 Amp in 100 ml NS slow IV/BD
